Surf Goddess: This 7" was released in 1995 and issued in a card stock sleeve. It was also available as a CDEP in a cardboard sleeve. Also shown is a scan of the original artwork.
 
 
 
NOTE: To the best of my knowledge, this 7" was NOT issued with a paper sleeve. I simply put the record in one for the purpose of scanning it.
